Survey Title
Add the title of your survey on this line. For example: Company ABC Employee Opinion Survey

Survey Logo
You can use this line to add your organization's logo.  You can add a logo by using a URL (web address) - for example: http://www.yourdomain.com/logo.jpg
You can also choose to upload a file from your computer by clicking on Choose File in the Upload Survey Logo area.

Helpful Hint: To pull a logo directly from a web-site, go to the website and "right-click" on the logo using your mouse. Under Properties the address/URL for the logo will be shown. Or you will see an option to Copy Image Address. You can copy and paste this address into the field provided.

Display Logo: You can chose to display your logo on any of the screens listed. Check or uncheck the boxes as needed.

Setting Survey Colors and Fonts Sizes:
You can set colors for various elements of your survey. The options are listed below. Click here for a safe web color guide with color numbers.
You can also set a default font size for the Title, Questions and Answers. Font sizes are in pixels (px), the standard measure for the web. As an example, if you are used to working in a word processing tool, like Word, 16px is equal to 12pt and 24px is 18pts.

Background Color
Set the color of the survey background.

Title Font Color
Set the color of the survey title.

Title Bar Color
Set the color of the Title bar at the top of the survey.

Title Font Size
Set the size of the Title font

Question Font Color
Set the color of the Question font.

Question Bar Color
Set the color of Question bar on which the Question text appears.

Question Font Size
Set the size of the Question font

Answer Font Size
Set the size of the Answer font

Progress Bar Color
Change the color of the Progress bar if it appears at the bottom of the screen.

Survey Instructions
These instructions will appear on the first page of your survey. Below are sample instructions:

Welcome to Company ABC’s Employee Opinion Survey. This survey is designed to measure your opinions about the quality of worklife at ABC company. Your individual responses will remain completely anonymous and confidential. Only group-level results will be reported to management. This survey should take only 10-15 minutes to complete. If you have any questions about this survey please contact Joe Smith at (###-###-####) or by e-mail at jsmith@companyabc.com.

Final Instructions
These instructions will appear on the last page of your survey. Below are sample instructions:

Thank you for participating in Company ABC’s Employee Opinion Survey. If you would like to visit our web-site go to www.companyabc.com, or you may close your web-browser and return to work. Thanks Again!

Survey Footer
This text will appear at the bottom of every page. You may want to include additional instructions, a disclaimer about confidientiality, or any other text. Sample footers include:
